Joining a growing team of clinical staff and peer support staff, you will provide essential operational support to the Kinship PATHway service including initial post-referral liaison with individuals and families. 

The role is available on a part-time or full time basis (21 to 35 hrs per week) and is home based (anywhere in the UK), with working pattern negotiable.

Salary is £27.917 full time, pro-rata for part time.

MAIN DUTIES AND RESPONSIBILITIES

  • To work with the PATH administrator to ensure the processing of referrals is accurate and timely

  • To create and maintain family records in the organisation's digital case management systems.

  • Undertake Initial meetings with families referred and attend and contribute to Case Management and Support discussions with Clinical staff.

  • Liaise with referred individuals and families to manage service access and service user expectations.

  • To liaise with referring social worker and/or any other relevant agency regarding service referrals.

  • Carry a small caseload.

  • When necessary, can support therapeutic group work and co-deliver training.

  • To contribute to and maintain accurate records for those using the service, on Adoption UK systems and ensuring compliance with both GDPR, safeguarding and confidentiality.

  • Be effective in gathering service data.

  • Contribute to team meetings and events, sharing knowledge and experience.

  • Develop a knowledge of support services available to families from Adoption UK and other organisations, as well as current policy and practice, and impart that information to the families on how best to access these services.
